Submit the following:

- ✅ Immunization Record (Florida DH Form 680)
- ✅ Recent Physical Exam (within the last year)
- ✅ Completed Registration Forms
Returning Students
Grade-Specific Updates Only:
- 4th Graders: Updated physical exam
- 7th Graders: Updated immunization record (DH Form 680)
- All Grades: Completed registration forms
